Seit Einführung des Cochlea-Implantats (CI) gab es Anstrengungen das Hörergebnis zu verbessern. Die Wahl der Elektrodenlänge des CI ist hierfür ein entscheidender Parameter. Mit Verfügbarkeit von CI-Elektroden verschiedener Längen ist eine personalisierte Anpassung möglich geworden. Die individuelle Bestimmung der Länge der lateralen Wand der Cochlea (LDC-LW) kann mit einer 3D-Rekonstruktion aus dem Felsenbein-CT erfolgen. Alternativ kann im CT die Distanz vom runden Fenster durch den Modiolus zur lateralen Wand der Gegenseite gemessen (A-Wert) und hieraus die LDC-LW berechnet werden. Ziel der Arbeit war es, die Ergebnisse beider Methoden zu vergleichen. Es zeigte sich eine hohe Korrelation des A-Wertes mit der LDC-LW und es konnte kein signifikanter Unterschied zwischen gemessener und errechneter LDC-LW nachgewiesen werden. Die präoperative Berechnung der LDC-LW mithilfe des A-Wertes ist gut zur Auswahl des CI geeignet, da sie schnell und einfach im klinischen Alltag anwendbar ist.
Since the introduction of the cochlear implant (CI), efforts have been made to improve hearing outcomes. The choice of electrode length of the CI is an important parameter for this. With the availability of CI electrodes of different lengths, a personalized adjustment has become possible. The individual determination of the length of the lateral wall of the cochlea (LDC-LW) can be done with a 3D reconstruction from a CT of the petrosal bone. Alternatively, the distance from the round window through the modiolus to the lateral wall of the opposite side (distance A) can be measured and the LDC-LW can be calculated. The aim of this work was to compare the results of these two methods. A high correlation of distance A with the LDC-LW was shown with no significant difference between measured and calculated LDC-LW. The preoperative calculation of the LDC-LW using distance A is well suited for selecting the CI, as it is quick and easily applicable in everyday clinical practice.
