The new member states of the European Union often experience new challenges with drug use in prisons and related health problems like high prevalence of infectious diseases. This study analyses the situation of prison drug use, infectious deseases and health care services in four countries: Estonia, Hungary, Lithuania, and Poland. Based on a comprehensive literature review the prison health care in the four countries is described. A detailed picture is given of health status, drug use, health care and drug services in prisons. <engl.>

Die Arbeitsbedingungen der Bediensteten in Justizvollzugsanstalten sind geprägt durch ein hohes Maß an Problemen: Überbelegungen der Haftanstalten, interkulturelle Konflikte, die sich häufig gewaltsam entladen, Bandenkriminalität innerhalb der Anstalten, Sprachprobleme im Umgang mit Inhaftierten, Drogenmissbrauch, bauliche Überalterung der Haftanstalten und häufig unzureichende Personalausstattungen mit einem defizitären Fortbildungsstand, Unter- und Überforderung. Diese Bedingungen führen zu physischen und psychischen Belastungen und machen das Berufsfeld in diesem Sinne einzigartig. Beispielhaft anhand der Justizvollzugsanstalt Bremen-Oslebshausen wurde mit einer MitarbeiterInnen- und Inhaftiertenbefragung untersucht, welche Faktoren die Ursachen für Stress und Belastungen im Gefängnis bilden. <dt.>

Drug dependence has to be treated as a severe disease and everyone has a part to play to ensure the best treatment for prisoners and also to ensure that drug related harm is kept as low as possible. Applying the recommendations in this guide will contribute to a healthier prison for prisoners with drug dependence with satisfying roles for staff members and a marked reduction in the harm that drug use in prisons can create. This publication is based on the expertise of scientists and medical doctors, psychiatrists, healthcare professionals working in the field of substitution treatment in prisons. Relevant international literature and databases have been reviewed in order to develop the best evidence based guidance. The key aim of this research was to provide an overview of the legislation, policy and practice concerning harm reduction services provided for problematic drug users (PDUs) in 9 European Union (EU) countries. To achieve this, the main objectives were to analyse international, national policies on harm reduction; to explore how harm reduction in conceptualised in different cultural contexts; to identify existing harm reduction initiatives in prisons; to identify the obstacles and barriers that need to be to overcome in order to implement harm reduction measures in prisons; to examine in detail the policies and harm reduction services in place in two sample institutions; to identify models of best practice and promote awareness of the harm reduction initiatives operating in the area of problematic drug users in custody, and finally to present examples of harm reduction measures in prisons from each of the sample countries. Key requirements needed in order to successfully implement sustainable harm reduction services in prisons are identified. These requirements can be formulated on all relevant levels: attitude towards and knowledge about harm reduction, drug addiction and health risks for all key actors, necessary changes both on the policy and practice level. Guidelines are formulated from evidence-based practice, as opposed to moral and value judgements. <engl.>
